Membership Criteria
- Applicants must be based in Nevada.
- Applicants must have a self-funded health plan.
- Applicants must agree to pay annual dues (amount is based on the number of employees/participants of your company).
- Applicants must sign a Participant Agreement.
- Applicants are subject to Board approval.
